I have read and understood the medical note and safety guidelines at the top of this page and completed answers accurately to the best of my knowledge. I am not aware of any reason that I should not participate in Pilates sessions. I understand that it is my responsibility to inform the instructor if I experience any new or unusual symptoms during or following sessions.
